We offer short-term care after a hospital stay, to give a family member support or to help you adjust at home after you’ve been unwell. We also offer long-term support as often or as little as you need it.

Joined-up care
Keeping detailed notes is a crucial part of good care delivery at home. We’ve invested in an electronic system that allows everyone in your Vitality Home Health care team to record, share and view information about your care as it happens. These notes can then be safely shared with your GP and anyone else in your healthcare team, such as a specialist nurse, occupational therapists or physios.
We can also share your information with your family, if you wish. We understand how important it is to keep your loved ones updated so they feel involved in your care every step of the way.
